Independence and Wellbeing Practitioner

This is great opportunity to join Suffolk County Council as an Independence and Wellbeing Practitioner working from Hartismere Hospital, Eye.
This is an exciting opportunity to represent Adult Social Care and work alongside the people of Suffolk, their families, and advocates as well as other professionals and agencies. You will be reviewing people in hospital and facilitating their hospital discharges.
Your role and responsibilities
We are looking for enthusiastic practitioners with a positive attitude. You will work in a interesting environment as part of a busy integrated team, with other health professionals.
You will play a key part in promoting personalisation, independence, wellbeing and choice by providing a social care service for people. You will be expected to exercise a high level of autonomous professional judgement and risk evaluation, taking responsibility for the safety and well-being of people accessing adult and community services within a health environment.
The role will include completing asset-based assessments/reviews, short term retirement plans, care and support plans and safeguarding plans in line with Supporting Lives, Connecting Communities and using a Signs of Safety and Solution Focused approach.

- excellent communication skills with the people we are working alongside, their families and/or advocates as well as other professionals
- the enthusiasm to contribute to a multi- agency team approach for responding within urgent timescales
- resilience under pressure and the ability to remain positive when challenged.

The team
The team seeks to improve the quality of life and independence of the people we work with. Within the hospital we continue to develop changes to discharge processes. These will minimise a person`s stay in hospital whilst maximising the opportunities our teams have to work with people in a community setting focussing on reablement and improving their health and long-term wellbeing.
