A great opportunity to join Suffolk County Council`s West Mildenhall Team as an Independence and Wellbeing Practitioner, working from West Suffolk House.

Your role and responsibilities
We are looking for an enthusiastic practitioner with a positive mindset to join the Mildenhall Care Team in the West of Suffolk.
Your role will see you work alongside colleagues both within the organisational and outside, including health colleagues, allocations lead, Business, Finance and Service Development and Contracts teams and wider partners, putting you in an ideal position to develop, build and help your experience in Adult Social Care.
The main purpose of this role is to work with adults to identify and deliver good quality outcomes to support and maximise independence and wellbeing.
You will need
- experience of creating reablement plans in line with a signs of safety solution focused approach
- good standard of education
- experience of working with people with disabilities.
